Blood cultures before antibiotics

There are many reasons to get multiple sets of blood cultures before antibiotics- the main one is because it impacts patient care. I once took care of a patient who had received prolonged empiric antibiotics from a provider who hadn’t checked blood cultures beforehand. She eventually ended up being diagnosed with culture-negative endocarditis and needed cardiac surgery for severe regurigation. This could have perhaps been avoided if she had received blood cultures before antibiotics.
